CES/SLS Respite $24.00/HR
Short-term care due to the absence of the individual’s normal care provider.
SLS/DD Supported Community Connector Based on Service Level
Assisting the individual with accessing typical activities and functions in the community.
CES Community Connector $25.00/HR
Enable the individual to access typical activities and functions of the community.
CES/SLS Extraordinary Cleaning $28.00/HR
Enhanced cleaning support beyond routine household tasks, tailored to individual needs and service plans.
Non Medical Transportation $10 – $25/TRIP
Transportation support based on the appropriate tier.
DD – IRSS Based on Service Level
